 Directions and Forms for PIAA High School Sports:

1. Print and fill out the Parent Insurance Form (Parent Insurance Form)
2. Print and fill out pages 1-5 of the PIAA Physical Examination Form (PIAA Physical Exam Form)
3. If your child has already participated in a sport in the current school year (2012-2013), print and fill out a Recertification form (Re-certification form)
4. Sport Parent Code of Conduct (Code of Conduct)
5. Send all completed forms to the school address listed
